Breast Health Questionnaire

ARE YOU
– over 40 years of age?
– post or perimenopausal?
– on hormone replacement therapy?

DO YOU
– have a family history of breast disease or cancer e.g. sister, mother or daughter?
– have a painless lump in the breast?
– have an abnormal thickening of the breast tissue or change in the density of the breast?
-have a lump under the arm (axilla/armpit)?
-have a redness of the skin overlying the breast?
-have a nipple discharge?
-have dimpling of the skin?
-had a child after 30 years of age or not having children?

If you answer yes to any of the above, you require a breast examination and in the most cases imaging of the breasts in form of a mammogram (breast-x-ray).
